Membership Suspension & Cancellation Policy

By submitting this form, you acknowledge and agree to the following:

• All suspension or cancellation requests must be submitted through this form. Requests made verbally, by text, or on social media will not be accepted.

• Membership cancellations must be submitted at least 30 days prior to the next billing date to avoid being charged for the next billing cycle.

• Memberships remain active and billable until the request is officially processed and confirmed by Ebb N Flo Maui.

• Suspension requests are for temporary holds only and are subject to approval by Ebb N Flo Maui.

• Vacation suspensions may be granted for up to 1 month.

• Medical suspensions may be granted for up to 3 months maximum and must include proof.

• Extensions may be granted only under special circumstances and only with written approval from Ebb N Flo Maui.

• Cancellation does not void any outstanding balances or prior commitments.

• No refunds will be issued for unused time, missed classes, or partial billing periods.

• You will receive a confirmation email once your request has been reviewed and processed.
